Centerstone is seeking a Community Health Worker to improve health and well-being by bridging gaps between healthcare providers and community members. This role focuses on outreach, education, and navigation, often working evenings or weekends to fit community schedules.

The CHW will collaborate with interdisciplinary teams to empower patients, address social determinants of health, and ensure access to resources and services across the community.

Qualifications

• High school diploma or equivalent required.
• Certification as a Community Health Worker preferred or willingness to obtain.
• Valid Indiana Driver’s License.

Responsabilités

• Conduct community outreach and engagement to identify those in need of health services.
• Provide culturally relevant health education on chronic disease, preventive care, mental health, nutrition.
• Help patients navigate healthcare systems, schedule appointments, and follow up on care plans.
• Coordinate care with housing, food, transportation and social services; maintain pantry records.
