Community Health and Wellness of Greater Torrington, Inc. seeks a Community Health Worker (CHW) to assist patients and families in navigating community and social services, promoting healthy behaviors, and coordinating care with providers and the care management team.

The CHW will participate in outreach, home visits, health screenings, and referrals, while maintaining patient records and supporting the care team in a collaborative environment. English proficiency required; Spanish preferred.

Responsabilidades

• Create connections between vulnerable populations and healthcare providers to ensure comprehensive, coordinated care.
• Work cooperatively with clinical staff assigned to the same patient.
• Manage assigned caseload of patients.
• Foster positive relations with patients, staff, and community partners.
• Assist patients in navigating health care and social service systems.
• Connect patients to transportation resources for appointments.
• Assist with completing applications, registrations, and forms.
