Job summary

Reema Health, Inc. is seeking a Community Health Navigator to work remotely in the United States. You will build relationships with members, guide them toward greater engagement in their healthcare, and help address social barriers using Reema's platform.

This role requires travel 25–50% in a 30-mile community service area and a valid driver’s license with insurance. You will conduct outreach, enroll members in our care model, manage a sizeable caseload, and coordinate referrals and appointments

Qualifications

• Must be able to find or improvise a path forward when barriers arise.
• Strong verbal and written communication skills.
• Comfort learning and troubleshooting new tools on mobile and desktop.
• Self-motivated to stay productive on a remote caseload.
• Commitment to collaboration and team building in a remote environment.

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members) and cover for others during spikes.
• Assess member needs and social determinants of health to build care plans.
• Establish trusting relationships and follow through in a timely manner.
• Help members understand health activities and overcome barriers to care.
• Coordinate referrals and appointment scheduling and attendance.
• Organize workload to hit monthly goals and daily metrics.
• Engage with members via phone, text, email, and in-person visits.
