Job summary

Reema Health is hiring a Community Health Navigator to build trusted relationships with community members and guide them toward greater engagement in their healthcare. You will work via the Reema platform to schedule visits, document progress, and meet monthly goals while adapting to evolving program needs.

You will travel within a 30-mile community service area, with 25–50% travel, and support members through social determinants of health, coordinating referrals and transportation as needed.

Qualifications

• Strong verbal and written communication.
• Comfortable learning and using new tools on mobile and desktop.
• Self-motivated to stay productive on a remote caseload.

Responsibilities

• Conduct outreach and enroll members in care model.
• Manage a caseload and collaborate with teammates.
• Coordinate referrals and schedule/track appointments.
• Document work in the Reema App and maintain care plans.
