Job summary

Reema Health, Inc. is seeking a Community Health Navigator to build trusted relationships with members and guide them toward proactive engagement in their health. You will work with our care model, scheduling visits, documenting progress in the Reema App, and coordinating referrals while maintaining HIPAA-compliant practices.

Expect a dynamic, startup environment with evolving roles. You will travel 25-50% within a 30-mile community area, work Monday-Friday with occasional evenings or weekends,

Qualifications

• 18 years of age or older.
• Available to work Monday-Friday during core business hours.
• Valid driver's license, insurance, and reliable transportation.
• Pre-employment drug screening and background check required.
• HIPAA compliance and protecting patient information.
• Daily use of technology-based tools and systems.
• Lift and carry up to 25 lbs occasionally.

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members) and provide coverage for others during absences.
• Assess member needs, including social determinants of health, to create member-centered care plans.
• Establish trusting relationships and follow through in a timely, compassionate manner.
• Help members understand why a health activity matters and address hesitations with empathy.
• Coordinate referrals and schedule appointments, ensuring attendance.
• Organize workload to hit monthly goals and daily metrics.
• Engage with members via phone, text, email, and in-person visits.
