Job summary

Reema Health, Inc. seeks a Community Health Navigator to build trusted relationships with members and guide them toward better engagement in their own healthcare. You will work with members via Reema's platform to schedule visits, document progress, and address social barriers in a growing startup environment.

The role requires outreach, caseload management (80-120 members), and collaboration with a multidisciplinary team to ensure monthly goals are met and care plans stay current.

Qualifications

• Must be able to adapt and solve barriers for members.
• Strong verbal and written communication skills.
• Comfort with learning and using new software/tools.
• Self-motivated to manage a remote caseload with minimal oversight.
• Able to collaborate with a multidisciplinary team.

Responsibilities

• Conduct outreach via phone and in-person visits; enroll members in care model.
• Manage a caseload and cover for colleagues as needed.
• Assess social determinants of health to build member-centered plans.
• Coordinate referrals and schedule appointments; track progress monthly.
• Organize workload to meet monthly goals and daily metrics.
