Job summary

UnitedHealth Group is seeking a Quality Community Health Worker (CHW) to bridge members with care teams and providers. The role focuses on proactive outreach to close gaps in care, promote preventive services, and navigate follow-up after hospitalizations, with travel within the community.

Qualifications

High School Diploma or GED required.
2+ years in community health within healthcare.
Willingness to travel up to 50% within assigned region.
Must reside in Nebraska and have a valid Nebraska driver’s license.
Proficient in Microsoft Outlook, Teams, Excel, Word.

Responsibilities

Conduct targeted outreach to members identified through claims data and reports to drive quality improvements.
Promote follow-up appointments after ED visits and hospitalizations, including behavioral health cases.
Collaborate with internal teams to implement PIP strategies and document outcomes.
Support implementation of quality improvement strategies to enhance plan performance and outcomes.
Assist members in navigating the healthcare system, including scheduling appointments and understanding benefits.
Document outreach activities and outcomes in designated systems.
