Job summary

Community Health Plan of Washington is seeking a Community Health Worker to serve as a liaison between health and social services and the community. The role focuses on outreach, education, advocacy, and improving access to services with a strong emphasis on equity and culturally competent care.

Qualifications

• Three years of experience delivering medical/mental health/substance abuse services to individuals with chronic conditions or severe mental illness.
• Comfortable working with at-risk populations and diverse communities.
• Experience using a care management or EMR system.
• High school diploma or equivalent; Peer Specialist certification if needed.
• Valid driver’s license and reliable transportation.

Responsibilities

• Engage potential care management participants identified through data and referrals.
• Provide outreach including home visits and information on community resources.
• Follow up with clients by phone or in person to support appointments and medication adherence.
• Identify referrals to community and network providers via outreach.
• Share progress updates with care managers and providers as needed.
• Maintain case documentation per policies and procedures.
• Organize activities that promote socialization, recovery, and independence.
• Educate enrollees to improve health knowledge and adherence.
