Job summary

UnitedHealthcare is seeking a Quality Community Health Worker (CHW) to serve as a bridge between the health plan, providers, and Medicaid members in Nebraska. This remote role emphasizes outreach to close care gaps, support preventive screenings, navigate services, and promote health education across diverse communities.

Travel up to 50% within the community is required; you will meet members in homes, clinics, or community settings and document outcomes in designated systems.

Qualifications

• High School Diploma/GED required.
• 2+ years in community health care setting.
• Proficient with Outlook, Teams, Excel, and Word.
• Willingness to travel up to 50% within Nebraska.
• Must reside in Nebraska with valid driver’s license.
• Access to reliable transportation.
• Internet access at home with high-speed connection.

Responsibilities

• Conduct outreach to drive quality improvements and close care gaps.
• Promote follow-up after ED visits and hospitalizations.
• Collaborate to implement PIP strategies.
• Document outcomes in designated systems.
• Support community outreach and education initiatives.
• Provide culturally appropriate health education.
• Assist members in navigating care and benefits.
