Job summary

Qualifications

• High School Diploma/GED required.
• 3+ years of experience in Community Health or related field.
• Proficiency with Outlook/Teams/Excel/Word and multiple web apps.
• Must reside in Nebraska and have reliable transportation for regional travel (up to 25%).

Responsibilities

• Assess, plan, and implement care strategies individualized for members.
• Create a positive experience and relationship with the members.
• Collaborate with members, family, and healthcare providers.
• Identify and initiate referrals for social service programs and resources.
• Document all member assessments, care plans, and referrals.
• Engage members face-to-face or telephonically and adhere to documentation standards.
• Partner with care teams and stay informed about community cultures and values.
• Perform other duties as assigned.
