Job summary

Qualifications

• Residence in LA within Region 9 (St. Tammany, Livingston, Washington, Tangipahoa).
• Able to travel 60-75% with reliable transportation.
• 3-5 years' relevant work experience.
• 1-2 years' experience providing clinical support services.
• 1 year of community outreach experience and knowledge of LA resources.
• 1 year' experience working with clients with clinical needs.

Responsibilities

• Conduct home visits to reinforce care plans and adapt to member culture.
• Educate providers on social determinants of health and care opportunities.
• Develop positive relationships with members, providers and staff from diverse backgrounds.
• Assist with care plan implementation and development of health/social management plans.
• Identify socio-economic issues affecting health and facilitate access to services.
• Enter and maintain member records in the EHR and complete program documentation.
• Travel extensively to client homes and outreach locations.
