Job summary

Benefis Health System is seeking a Home-Based Care Manager in Montana. The role focuses on providing psychosocial assessments, coordinating services, and supporting Medicaid beneficiaries living in their own homes or communities.

You will join a multidisciplinary Care Management Team and help plan, coordinate, and monitor services to improve quality of life, with deadlines and pressure handled professionally. Eligibility includes a Bachelor's in social work or related field.

Qualifications

• Bachelor's degree in social work or related field.
• Three years of social work experience in health care preferred.
• Knowledge of case management methods and procedures.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of long-term care consumer issues.
• Experience with budget management.
• Understanding of human behavior, disabilities, and aging.

Responsibilities

• Perform psychosocial assessments and coordinate services for Medicaid beneficiaries.
• Coordinate with the Care Management Team to plan, implement and monitor individualized care.
• Assist in navigating housing, finances, and community resources.
• Demonstrate ability to handle pressure, deadlines, and changing situations.
