• Knowledge of long-term care consumer issues and aging.

Responsibilities

• Perform psychosocial assessments and coordinate services to meet beneficiary needs.
• Support housing, financial stability, and community resource navigation.
• Coordinate with Care Management Team to plan, implement, and monitor care.
• Remain compliant with licensing board rules and Benefis Health System policies.
