Job summary

Benefis Health System in Montana seeks a Home and Community‑Based Case Manager to provide psychosocial assessments, care planning, and coordination of services for elderly or disabled Medicaid beneficiaries living at home or in the community.

You will be part of the Care Management Team, addressing housing, financial stability, and community resources while aiming to improve quality of life and provide emotional support in a dynamic, deadline-driven environment.

Qualifications

• Bachelor's degree in social work, psychology, sociology, or related field.
• Three (3) years of social work experience in a health care setting preferred.
• Knowledge of case management methods, practice, and procedure.
• Knowledge of diagnostic and crisis intervention skills.

Responsibilities

• Perform psychosocial assessments and connect clients to mental health, housing, financial stability, and community resources.
• Coordinate psychosocial services as part of the Care Management team to improve quality of life and emotional support.
• Support elderly or physically disabled Medicaid beneficiaries in home/community settings and assist with long‑term care planning.
