Job summary

Benefis Health System is seeking a Home and Community-Based Case Manager to provide psychosocial assessments, coordinate services, and support Medicaid beneficiaries in their own homes or communities.

The role focuses on meeting psychosocial needs, coordinating care, and connecting individuals with housing, financial stability, and community resources, under the Care Management Team. Compliance with policies and licensing requirements is essential.

Qualifications

• Bachelor’s degree in social work or related field.
• Three years of social work experience in a health care setting preferred.
• Knowledge of case management methods and procedures.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long-term care consumers.

Responsibilities

• Perform psychosocial assessments and help coordinate services as part of the Care Management Team.
• Assist with housing, financial stability, and community resource navigation for beneficiaries.
• Support individualized member-driven planning and monitoring of services.
• Adhere to regulatory requirements and Benefis Health System policies.
