Job summary

Benefis Health System is seeking a Home and Community-Based Case Manager to provide psychosocial assessments, support mental health needs, and coordinate services for Montana Big Sky Waiver beneficiaries living at home.

As part of the Care Management Team, you will plan, coordinate, assess, and monitor individualized care, navigate housing and financial resources, and ensure smooth long-term care placements in home and community settings.

Qualifications

• Bachelor’s degree in social work or related field.
• Three years of social work experience in healthcare preferred.
• Knowledge of case management methods and crisis intervention.
• Understanding of long-term care needs, aging, and disabilities.
• Experience budgeting and managing resources.

Responsibilities

• Provide psychosocial assessments and mental health support.
• Coordinate and monitor home/community-based care for seniors and disabled.
• Assist with housing, finances, and community resources.
• Participate in Care Management team planning and collaboration.
• Support long-term care placements in home/community settings.
