Job summary

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or related field.
• Three years of social work experience in a health care setting (preferred).
• Knowledge of case management methods, practice, and procedure.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long-term care consumers.
• Knowledge of managing a budget.

Responsibilities

• Work as part of a multi-disciplinary Care Management Team to plan, coordinate, assess, and monitor services for Montana Big Sky Waiver Medicaid beneficiaries in home/community settings.
• Provide psychosocial assessments, mental health support, housing, financial stability, and community resources.
• Improve quality of life and coordinate psychosocial services.
• Demonstrate ability to work under pressure to meet deadlines and adapt to changing situations.
• Handle a variety of people, stressful situations, and conflict.
• Perform all duties in line with Benefis Health System policies.
