Benefis Health System in Montana seeks a Home and Community‑Based case management professional to provide psychosocial assessments, coordinate services, and support elderly or physically disabled beneficiaries in their homes and communities.

The role focuses on psychosocial needs, housing navigation, and community resources within a Care Management team, aiming to improve quality of life while adhering to system policies.

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or related field.
• Three years of social work experience in a health care setting (preferred).
• Knowledge of case management methods, practice, and procedure.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long-term care consumers.
• Knowledge of managing a budget.

Responsabilités

• Work as part of a multi-disciplinary Care Management Team to plan, coordinate, assess, and monitor services for Montana Big Sky Waiver Medicaid beneficiaries in home/community settings.
• Provide psychosocial assessments, mental health support, housing, financial stability, and community resources.
• Improve quality of life and coordinate psychosocial services.
• Demonstrate ability to work under pressure to meet deadlines and adapt to changing situations.
• Handle a variety of people, stressful situations, and conflict.
• Perform all duties in line with Benefis Health System policies.
