Job summary

Benefis Health System is seeking a Home and Community-Based Case Manager to deliver psychosocial assessments and coordinate services for Medicaid beneficiaries in their homes and communities. Qualifications

• Bachelor’s degree in social work or psychology, sociology, or related field.
• Three (3) years of social work experience in a health care setting preferred.
• Knowledge of case management methods, practice, and procedure.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long-term care consumers.
• Prior knowledge of managing a budget.
• Knowledge of human behavior, disabilities, and the aging process.

Responsibilities

• Provide psychosocial assessments and care planning within home/community settings.
• Coordinate services to improve quality of life and emotional support for beneficiaries.
• Collaborate with the Care Management Team for individualized planning and monitoring.
• Follow licensing board requirements and Benefis Health System policies.
