Job summary

Benefis Health System is seeking a Home and Community-Based Case Manager to coordinate psychosocial assessments, planning, and monitoring for Medicaid beneficiaries living in their homes and communities.

You will work with a multidisciplinary Care Management Team to provide housing navigation, financial support, and access to community resources, aimed at improving quality of life and timely service delivery.

Qualifications

• Bachelor’s degree in social work or related field.
• 3 years of social work experience in health care preferred.
• Knowledge of case management methods and procedures.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of long-term care consumer needs.
• Budget management knowledge.
• Understanding of human behavior, disabilities, and aging.

Responsibilities

• Provide psychosocial assessments and coordinate services.
• Navigate housing, financial stability, and community resources.
• Collaborate with Care Management Team for planning and monitoring services.
• Adhere to licensing board requirements and Benefis policies.
