Job summary

The Columbus Organization in Georgia is seeking a Part-Time Care Coordination Supervisor to support Intensive Support Coordinators and manage a caseload of ISC participants, with at least one annual face-to-face visit per participant.

The Clinical Supervisor will oversee a supervisory caseload of up to 100 ISC participants, providing clinical consultation and monitoring adherence to care plans and ISC protocols. Evening/weekend work may be required based on demand.

Qualifications

• Minimum 2 years of professional experience with individuals with complex behavioral issues in a behavioral health setting.
• Minimum 3 years’ experience providing home and community-based case management for individuals with disabilities.
• Two years of experience making recommendations as part of a client's service plan (clinical treatment, counseling, or determining eligibility).

Responsibilities

• Provide ongoing consultation to ISCs including joint visits and risk assessment.
• Perform quarterly caseload reviews (25%) to ensure protocol adherence.
• Identify training needs with Team Leaders and Clinical Director and develop staff plans.
• Act as clinical consultant for ISC Coordinators and participants.
• Conduct at least one annual joint visit per participant on supervisory caseload.
• Maintain familiarity with care plans and evaluate efficacy of plans.
• Hold monthly supervisory sessions with ISCs and be a resource between sessions.
