Qualifications

• Bachelor’s degree in a human service field required.
• At least one year of experience serving persons with developmental disabilities in a case management or coordination role; Master’s degree may substitute.
• Mobile worker: regular travel to different work locations is essential and you must reside in the assigned territory.

Responsibilities

• Assist individuals with intellectual challenges or developmental disabilities to access and coordinate needed support and services in the community.
• Coordinate and facilitate information gathering for the support plan, arrange assessments, and review recommendations prior to ISP meetings.
• Assist individuals and families in applying for Medicaid eligibility and access community resources to obtain needed services.
• Monitor Medicaid redetermination dates, complete necessary paperwork, and maintain eligibility documentation.
• Oversee participation in ISP services and ensure outcomes are met; maintain monthly face-to-face contact and document all contacts.
