Job summary

CareStar, Inc. is seeking an Intensive Support Coordinator to serve Jackson County, GA, and surrounding counties.

This full-time role offers remote work with field visits; you’ll assess needs, develop personalized care plans, and connect participants with medical, social, and supportive services to promote healthier, more independent lives. You will work with a mission-driven team, follow CareStar policies, maintain confidentiality, and travel as needed.

Qualifications

• Bachelor’s degree in a human service field.

Responsibilities

• Provides information and assistance to link participants with medical, social and/or educational providers and/or other programs and services that can provide needed services to address identified needs and achieve identified goals.
• Conducts health and safety, social and environmental assessments per established guidelines.
• Serves on work groups, community groups, committees, or other task forces and attends meetings as requested.
• Effective organizational and communication skills
• Experience providing care management/coordination preferred.
• Willingness and ability to travel and have reliable transportation, valid driver's license and car insurance as required by state law.

Education

Bachelor’s degree in human service field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing
