Qualifications

• Must have a minimum of a bachelor's degree in a human service field such as social work, psychology, or related field.
• At least one year of experience in serving persons with developmental disabilities in case management or coordination; Master’s degree may substitute for the required experience.

Responsibilities

• Assists people with intellectual challenges or developmental disabilities to access and coordinate needed support and services in community settings.
• Coordinate and facilitate information gathering for the support plan; arrange and obtain relevant assessments.
• Identify and access cost-effective services and community resources; assist Medicaid eligibility processes and annual redeterminations.
• Oversee participation in ISP-supported services and monitor alignment with client needs and goals.
• Maintain monthly face-to-face contact and document all contacts in the support notes; ensure timely reporting and HIPAA-compliant practices.
