CareStar, Inc. is seeking an Intensive Support Coordinator (ISC) for McDuffie County, GA and surrounding areas.

This full-time role combines remote work with required field visits to connect participants with medical, social, and educational services to address identified needs. You will assess needs, develop personalized care plans, coordinate services, and collaborate with communities to empower individuals toward healthier, more independent lives.

Qualifications

• Bachelor's degree in a human services field.
• Two years' direct service experience serving persons with DD/IDD or related disabilities.
• Two years' healthcare/rehabilitative settings experience.
• Experience completing assessments and service plan development and monitoring.
• Strong organizational and communication skills.

Responsibilities

• Link participants to medical, social, and educational services.
• Conduct health, safety, social, and environmental assessments.
• Participate in work groups and community meetings.
• Coordinate care plans and monitor service delivery.
• Travel as required by state law for field visits.
