Job summary

The Columbus Organization, LLC is seeking a Care Coordination professional to assist individuals with intellectual and developmental disabilities across community settings. This remote field-based role requires travel within GA to conduct client visits and coordinate services, with administrative work performed remotely when not in the field.

Qualifications

• Requires a bachelor’s degree in a human service field such as social work, psychology, or related field.
• At least one year of experience serving persons with developmental disabilities in case management or related role; Master’s may substitute.
• Mobile worker traveling across assigned service area with regular client visits.

Responsibilities

• Assist people with intellectual or developmental disabilities to access and coordinate needed services in community settings.
• Coordinate information gathering for the support plan and arrange pertinent assessments.
• Review clinical information and recommendations before ISP meetings for interventions.
• Identify cost-effective services and Medicaid eligibility and access community resources.
• Maintain documentation, monitor Medicaid renewal dates, and ensure proper paperwork.
• Oversee ISP participation and ensure annual reviews and updates.
• Ensure confidentiality and compliance with HIPAA and organizational policies.
