Job summary

The Columbus Organization, LLC is seeking a full-time Care Coordination professional in a remote-friendly role for Hall and Gwinnett Counties, GA. You will coordinate services for individuals with intellectual and developmental disabilities, and conduct client visits as needed.

Required is a bachelor’s degree in a human services field, 2+ years of relevant experience, strong communication, and data-management skills. Travel is essential; some evenings/weekends may be required.

Qualifications

• Minimum bachelor’s degree in human service field (social work, psychology, nursing, etc.).
• Minimum two years of experience serving persons with DD/IDD or related disabilities.
• Mobile worker requiring travel to various client sites within GA.
• Residence in assigned service area with reliable transportation.

Responsibilities

• Coordinate and facilitate activities for gathering information for the support plan.
• Arrange and obtain clinically pertinent assessments and reviews for interventions.
• Monitor service delivery and provide technical assistance on program and client issues.
• Maintain documentation and eligibility information for clients in charts.
• Communicate effectively with clients, families, and other professionals.
• Attend ISP meetings and adjust plans based on client needs and preferences.
