Job summary

Equity Staffing Group is seeking a Clinical Pharmacist to join a Clinical Engagement Services Center in the United States. You will conduct telephonic consultations, perform comprehensive medication reviews, and support disease-state management and MTM initiatives.

The role requires remote work with flexible hours; you will also review claims, identify gaps in care, and educate patients while maintaining confidentiality and HIPAA compliance.

Qualifications

• Current and unrestricted Pharmacist license in state of residence.
• Strong knowledge of MTM services and the 5 Core Elements of MTM.
• 1+ year remote/independent work experience in cross-functional teams.
• Ability to navigate MTM platforms and multiple databases.
• Proficiency in Microsoft Word, Outlook, and Excel.
• Excellent organization, documentation, and time-management skills.
• Ability to work flexible rotating shifts.

Responsibilities

• Conduct telephonic consultations and comprehensive medication reviews with members.
• Schedule pharmacist consultation appointments for members in clinical programs.
• Review prescription claims and clinical information to identify gaps in care, high-risk meds, adherence concerns, interactions, substitutions, and generics.
• Assess regimens for unnecessary meds, dosing issues, and adverse reactions.
• Provide patient education regarding medications and disease-state management.
• Communicate clinical recommendations to members' healthcare providers.
• Document patient care, interventions, and recommendations accurately.
• Maintain HIPAA compliance and protect patient confidentiality.
• Meet productivity and performance expectations.
• Participate in departmental initiatives and special projects.
