Job summary

Pharmacy Careers is seeking an MTM Pharmacist to optimize medications through comprehensive reviews and patient coaching. Roles offer remote options or flexible hybrid schedules, enabling meaningful clinical work from home or nearby facilities.

Qualifications

• PharmD or Bachelor of Pharmacy degree required.
• Ambulatory, hospital, community MTM experience is a plus; all licensed pharmacists welcome.
• Strong patient counseling, independence and clear documentation skills.

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
• Identify medication problems, interactions, adherence barriers and cost-saving opportunities.
• Collaborate with prescribers to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
