Job summary

Pharmacy Careers is seeking an MTM Pharmacist to conduct comprehensive medication reviews, coach patients on adherence, and improve outcomes, often from home or with flexible schedules. The role offers remote or hybrid options and meaningful clinical work that helps patients optimize their medications.

You will work with health plans and care teams to resolve therapy issues, document interventions, and ensure CMS MTM compliance.

Qualifications

• Licensed pharmacist with PharmD or Bachelor of Pharmacy degree.
• Ambulatory, hospital, community, or MTM experience is a plus.
• New graduates are encouraged to apply.

Responsibilities

• Perform comprehensive medication reviews and targeted reviews with patients.
• Identify medication problems, interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain compliance with CMS MTM program requirements and quality metrics.
