Job summary

PharmD Live, a leading telehealth provider, is seeking a Pharmacist to deliver MTM and medication management remotely to patients across Western and Central states. This role emphasizes proactive care, education, and coordination with healthcare teams to improve outcomes.

You will perform medication reconciliation, support chronic disease programs, and engage patients using motivational interviewing in a fully remote setup. Licensure in at least one targeted state is required.

Qualifications

• PharmD degree required.
• Active pharmacist license in at least one targeted state (CA, OR, WA, AZ, NM, NV, UT, ID, MT, WY, KY, OK, AR, LA, WV, NH, VT).
• Willingness to obtain additional licensure as required.

Responsibilities

• Perform Medication Therapy Management (MTM) interventions.
• Complete medication reconciliation and identify optimization opportunities.
• Provide patient‑centered education on medications and disease management.
• Support chronic care initiatives (e.g., diabetes, hypertension, COPD, CHF).
• Use motivational interviewing to improve adherence.
• Collaborate with providers on treatment planning and medication adjustments.
• Detect and resolve medication‑related risks and discrepancies.
• Ensure timely and accurate documentation of all encounters.
• Facilitate smooth care transitions and ongoing care coordination.
