Qualifications

• Education: PharmD or Bachelor of Pharmacy.

• Skills: Excellent patient counseling, independent work style, clear documentation.

Responsibilities

• Perform CMRs and Targeted Medication Reviews with patients.
• Identify medication-related problems, interactions, adherence barriers, and cost-savings.
• Collaborate with prescribers and care teams to resolve therapy issues.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM compliance and quality metrics.
