Job summary

Qualifications

• PharmD or Bachelor of Pharmacy required.
• Ambulatory, hospital, or community MTM experience is a plus; new graduates encouraged to apply.
• Excellent patient counseling and meticulous documentation are essential.

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
• Identify medication-related problems, interactions, adherence barriers, and cost savings opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain compliance with CMS MTM program requirements and quality metrics.
