Job summary

Qualifications
- PharmD or Bachelor of Pharmacy required.
- Licensed pharmacist in good standing; new graduates encouraged.
- Experience in ambulatory, hospital, community, or MTM is a plus.
- Excellent patient counseling and clear documentation.
Responsibilities
- Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
- Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
- Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
- Educate patients and caregivers on safe, effective medication use.
- Maintain compliance with CMS MTM program requirements and quality metrics.
