Job summary

Pharmacy Careers is seeking a MTM Pharmacist to conduct comprehensive medication reviews and coach patients on adherence. This role offers remote options or flexible hybrid schedules, enabling meaningful clinical work from home or a convenient workplace.

You will identify medication-related problems, collaborate with care teams, and educate patients while maintaining CMS MTM program compliance and quality metrics. Licensed pharmacists of all experience levels are encouraged to apply.

Qualifications

• PharmD or Bachelor of Pharmacy degree required.
• Licensure as a pharmacist in good standing; new graduates welcome.
• Experience in ambulatory, hospital, community, or MTM contexts is a plus.

Responsibilities

• Perform CMRs and targeted reviews with patients.
• Identify drug interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
