UnitedHealth Group is seeking a Clinical Pharmacist to join our Home Delivery team in a remote capacity. This role engages members and providers to improve adherence and outcomes while leveraging MTM and counseling expertise.

Qualified candidates hold a PharmD or Bachelor’s in pharmacy, hold active licenses in residence, and have 3+ years of pharmacist and patient counseling experience. Telecommuter policy applies for remote work.

Qualifications

• Pharmacy degree (PharmD) or Bachelor’s in pharmacy.
• Active and unrestricted pharmacist license in the state of residence.
• Active pharmacist licenses in AZ, KS, TX, NC (for CA remote eligibility).
• 3+ years of experience as a licensed pharmacist.
• 3+ years of patient consultation experience.
• Basic proficiency with Microsoft Office and Windows-based applications.
• Demonstrated strong verbal communication and patient counseling skills.

Responsibilities

• Conduct telephonic outreach with members to improve health quality outcomes and medication adherence.
• Engage members on pharmacy STAR measures and adherence barriers.
• Provide medication counseling and education to support positive health outcomes.
• Enroll eligible members in auto-refill and other adherence programs.
• Collaborate with physicians, pharmacies, and healthcare teams to optimize therapy.
• Perform MTM services, including comprehensive medication reviews.
• Respond to drug information requests from members and providers.
• Document and track member interactions and interventions.
