Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide direct medication counseling, adherence support, and clinical monitoring from a home-based setup. The role emphasizes patient interaction and a flexible remote schedule, with telephonic or video visits to support optimal outcomes.

The ideal candidate holds a PharmD or Bachelor of Pharmacy, Active U.S. pharmacist licensure, and experience in ambulatory care or MTM.

Qualifications

Education: PharmD or Bachelor of Pharmacy.
Licensure: Active U.S. pharmacist license (multiple state licenses preferred but not required).
Experience: Ambulatory care, MTM, or clinical preferred.
Skills: Strong communication, empathy, and ability to build rapport virtually.

Responsibilities

Deliver telephonic or video-based medication counseling and education to patients and caregivers.
Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
Document all interventions and communications according to organizational standards.
Support population-health and disease-management programs.
