Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to deliver telephonic or video-based medication counseling and MTM services from home. The role emphasizes patient interaction, adherence support, and clinical monitoring with flexible scheduling.

You'll work on population-health initiatives, collaborate with clinicians, and document interventions per standards. A PharmD or PharmD-equivalent, active US license, and ambulatory care experience are preferred for this remote role.

Qualifications

• PharmD or Bachelor of Pharmacy degree.
• Active U.S. pharmacist license.
• Ambulatory care or MTM experience preferred.
• Strong communication and ability to engage patients virtually.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
