Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telephonic or video-based counseling, MTM, and adherence support from home. You will collaborate with healthcare providers to optimize therapy and document all interventions for population health programs.

Ideal candidates hold a PharmD or Bachelor of Pharmacy and an active U.S. pharmacist license, with ambulatory care or clinical experience preferred.

Qualifications

• PharmD or Bachelor of Pharmacy.
• Active U.S. pharmacist license (multiple state licenses preferred).
• Ambulatory care, MTM, or clinical experience preferred; hospital and community pharmacists welcome to apply.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews for high-risk or complex patients.
