Job summary

Qualifications

• Master's Degree in Social Work required by state law.
• LMSW licensure aligns with state requirements.
• Experience in medical social work or case management preferred.

Responsibilities

• Oversee management of members with acute or chronic conditions to improve outcomes and reduce costs.
• Perform initial and periodic assessments for long term care or case management programs.
• Develop individualized case plans and coordinate care across providers and settings.
• Communicate updates with providers, physicians, pharmacists and care staff.
• Identify opportunities to ensure appropriate care within the member's benefit plan.
• Provide information and education on social issues, mental health, and community services.
• Develop programs to reduce hospital admissions and improve community integration.
• Coach junior colleagues in social work techniques and processes.
• Collaborate with community members and policymakers on client needs and awareness campaigns.
