Job summary

CVS Health is seeking a Clinical Care Coordinator to oversee long-term care and psychosocial case management for members with chronic conditions or birth defects. You will coordinate treatment plans, engage with care teams, and connect patients with community resources.

The role involves home-based care coordination with substantial travel (25–50%) across Wayne and Macomb Counties. A Master’s in Social Work and LMSW licensure are required to meet state standards.

Qualifications

• Master's Degree in Social Work required by state law.
• LMSW licensure required.
• Must be willing and able to travel 25%-50% in Wayne and Macomb Counties.

Responsibilities

• Coordinates care for members with long-term rehabilitation and psychosocial needs to prevent readmissions and placement in custodial care.
• Implements and coordinates case management, psychosocial and quality-of-life activities across the continuum of care.
• Develops and carries out strategies to integrate acute and long-term care services for members.
• Provides psychosocial interventions through resource identification and program development.
• Communicates with providers, primary care physicians, Medical Directors, pharmacists, and care management staff.
• Identifies opportunities to ensure care access under the member's benefit plan in home or community settings.
• Educates on social issues, mental health and available services to empower individuals and communities.
• Develops programs to reduce hospital admissions and improve health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community members and policymakers to address issues.
