CVS Health is seeking a dedicated Care Coordinator for a remote role in Michigan with 25-50% travel to Wayne and Macomb Counties. You will coordinate long-term care services, perform assessments, and develop care plans to prevent readmissions and improve outcomes.

The ideal candidate holds a Master’s in Social Work with LMSW and has strong communication skills to collaborate with healthcare teams and communities.

Qualifications

• Experience in coordinating care for long-term rehabilitation or psychosocial needs.
• Ability to assess member needs and develop care plans.
• Strong interpersonal skills to work with providers, patients, and families.

Responsabilités

• Coordinate comprehensive care for members with acute or chronic conditions.
• Perform initial and periodic assessments under supervision.
• Develop individualized case plans to improve outcomes and reduce admissions.
• Communicate updates with providers and care teams.
• Identify opportunities to ensure all needed care is covered by benefits.
• Provide information and education on social issues and available services.
• Develop programs to reduce admissions and improve community integration.
• Mentor junior staff in social work methods and processes.
