Job summary

CVS Health is seeking a dedicated Care Coordinator to manage members with long-term rehabilitation and psychosocial needs. You will coordinate case management, psychosocial interventions, and quality-of-life activities across care settings, with referrals to consultants, home visits, and community resources.

You will develop integrated care plans, collaborate with providers and care teams, and work to reduce readmissions and improve health outcomes for diverse populations across Wayne and Macomb

Qualifications

• MSW required by state law.
• LMSW licensure required.
• Willingness to travel up to 25–50% in Wayne and Macomb Counties.
• Strong problem-solving and decision-making skills.
• Basic digital literacy skills.
• Knowledge of medical terminology.
• Ability to handle sensitive information ethically and responsibly.
• Ability to function in a clinical setting with diverse cultural dynamics.

Responsibilities

• Coordinate care for members with acute or chronic conditions to improve quality of care and outcomes while reducing costs.
• Conduct initial and periodic assessments for Long-Term Care or disease management programs under limited supervision.
• Develop individualized case plans for at-risk members in case or disease management.
• Communicate updates with providers, primary care physicians, Medical Directors, pharmacists, and care management staff.
• Identify opportunities to ensure members receive care covered by their benefit plan in home or community settings.
• Provide information and education on social issues, mental health, and available services.
• Develop complex programs to reduce hospital admissions and improve community integration and health outcomes.
• Mentor junior colleagues in social work practices and processes.
• Collaborate with community members and organizations to address client needs and raise awareness.
