Job summary

CVS Health is seeking a qualified Care Coordinator to support members with long-term rehabilitation and psychosocial needs. You will coordinate case management across the continuum of care, perform assessments, and develop personalized plans to improve outcomes.

The role involves travel within Wayne and Macomb Counties and a 40-hour work week. The position requires a Master’s in Social Work and LMSW licensure where applicable, with strong communication and problem-solving skills in a clinical

Qualifications

• Must have MSW and be licensed as LMSW where applicable.
• Must comply with state requirements where job duties are performed.
• Willingness and ability to travel up to 25%–50% in Wayne and Macomb Counties.
• Basic problem solving and decision-making skills.
• Basic digital literacy and ability to work with diverse populations.
• Knowledge of medical terminology.

Responsibilities

• Coordinate care for members with long-term rehabilitation and/or psychosocial needs to prevent adverse outcomes.
• Conduct initial and periodic assessments for Long-Term Care or case/disease management programs.
• Develop individualized case plans and coordinate services across care settings.
• Communicate updates with providers, PCPs, Medical Directors, pharmacists, and care-management staff.
• Identify opportunities to ensure members receive all necessary care within their benefit plan at home or in community settings.
• Provide information, resources, and education on social issues and available services.
• Develop complex programs to reduce hospital admissions and improve health outcomes.
• Coach junior colleagues in social work techniques and processes.
• Collaborate with community members and organizations to address issues and raise awareness.
