Formación

• Master's Degree in Social Work is required by state law.
• LMSW licensure required for the role.
• Willingness and ability to travel up to 25%–50% of the time in Wayne and Macomb Counties.

Responsabilidades

• Coordinates care for members with acute or chronic conditions to improve quality of care, outcomes, and cost reduction.
• Performs initial and periodic assessments under limited supervision to determine treatment course.
• Develops individualized care plans for at-risk members in case or disease management.
• Communicates updates with providers, PCPs, Medical Directors, pharmacists, and care management staff.
• Identifies gaps to ensure members receive all care covered under their benefit plan in home or community settings.
• Provides information and education on social issues and available services to empower individuals.
• Develops programs to reduce admissions and improve health outcomes for enrolled patients and others.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community members and organizations to address community issues.
