Job summary

Minnesota Department of Health is seeking a Clinic Network Specialist to lead statewide planning and coordination that strengthens community clinical linkages across more than 440 clinic sites. You will provide expertise in operational planning, systems design, and evidence-based strategies to improve cancer and heart health screening access in Minnesota.

Qualifications

• Three (3) years of professional experience in health program coordination, public health systems planning, or clinic operations management.
• Experience with healthcare organizations or clinics to implement programs or improve service delivery.
• Experience developing or applying evidence-based strategies to support underserved populations.
• Experience utilizing program data to monitor performance and guide workflow changes.
• Ability to independently analyze complex problems and develop systematic solutions across diverse clinic environments.
• Strong communication and partnership-building skills with clinical and IT teams.
• Demonstrated ability to lead change management efforts in a multi-site public health program.

Responsibilities

• Lead planning, systems authority, and network training for the Sage clinic network of over 440 sites; drive long-range strategic planning and training initiatives.
• Coordinate clinical-community linkage strategies and cardiovascular screening workflows for SagePlus; align site-level activities with program goals.
• Coordinate health systems frameworks to support patient navigation and optimize EHR workflows for patient outreach and eligibility assessments.
• Design, execute, and evaluate innovative community-clinical linkage projects across the network; align protocols for better population health outcomes.
• Lead statewide training for the 440+ Sage providers and collaborate on data transfer frameworks with MNIT for future data moves.
