Partners in Digital Health is seeking an RN Case Manager for the Minnetonka Well on-site clinic. You will be the on-site clinical presence, delivering patient-centered care, conducting nursing visits, and coordinating services with the care team.

The role requires an active Minnesota RN license and 2+ years of relevant experience in primary or ambulatory care. The schedule is Monday–Friday, day shift, with a strong emphasis on care continuity and population health.

Qualifications

• Graduate of an accredited School of Nursing.
• Current, unrestricted RN license in the state of employment.
• 2+ years of nursing experience in primary/ambulatory care settings.
• Experience using electronic medical records and healthcare technology systems.
• Proven knowledge of nursing standards, ethics, and regulatory requirements.
• Strong communication and customer service skills.
• Ability to work independently and manage multiple priorities.

Responsabilités

• Serve as the on-site RN, creating a welcoming, patient-centered experience.
• Conduct nursing visits including biometric screenings, immunizations, health coaching, triage and nurse-led services.
• Coordinate care and communication among patients, providers, specialists, pharmacies, and community resources.
• Support prior authorizations and follow up on clinical orders, referrals, test results, and patient care needs.
• Provide education on preventive care, wellness, medications, and chronic disease management.
• Maintain accurate, timely, and compliant documentation in the EMR and related systems.
• Cross-train and assist with front desk and administrative duties when needed.
