Qualifications

• Graduate of an accredited school of nursing.
• Associate degree or diploma in nursing required.
• Maintain a current Minnesota registered nurse license; WI RN license required upon hire.
• Current BLS provider status per American Heart Association guidelines.

Responsibilities

• Assess patient care needs and develop, implement, and evaluate plans of care.
• Plan individualized education for patients and caregivers and ensure continuity of care.
• Coordinate and deliver nursing care in a timely, ethical, and culturally competent manner.
• Document care and education accurately in the EHR and reassess plans as needed.
• Collaborate with interdisciplinary team to determine necessary resources and improve patient outcomes.
• Provide tele health management as appropriate and participate in ongoing professional development.
