Job summary

Golden Care, based in San Diego County, is seeking a Client Services Manager to oversee an assigned client caseload from onboarding through ongoing care management. You will serve as the central communications hub for clients, families, caregivers, referral partners, and internal teams, ensuring timely follow-through and accurate documentation.

This remote role, based in La Mesa, CA, requires exceptional organizational, empathy, and leadership skills.

Qualifications

• Bachelor’s degree and a minimum of five years’ experience in home care, healthcare, senior services, or care management.
• Demonstrated leadership or team coordination experience is highly preferred.
• Exceptional communication, relationship-building, conflict resolution, and problem-solving skills.
• Ability to manage multiple clients, prioritize competing demands, and handle sensitive situations with professionalism and empathy.
• Strong organizational capabilities, meticulous documentation practices, and accountability for consistent follow-through.
• Comfort and proficiency with technology and client management software; familiarity with WellSky is advantageous.

Responsibilities

• Client Care Management: Conduct in-home assessments and develop personalized care plans; facilitate handoffs between teams for onboarding and initiation.
• Caregiver Support and Collaboration: Share observations, address concerns, and provide guidance to caregivers; escalate issues as needed.
• Scheduling and Care Operations Support: Coordinate caregiver assignments and communicate changes to Scheduling and Operations.
• Client Satisfaction and Quality Assurance: Maintain proactive client contact, document concerns, and drive improvements to retain clients and meet KPIs.
• Communication and Documentation: Ensure timely, accurate client records in WellSky and share updates with stakeholders within 24 hours.
