Job summary

A non-profit organization in San Diego seeks an Entry Level Care Manager to provide mobile outreach and engagement, advocate for clients, and coordinate care management. Responsibilities

  • Provide mobile outreach and engagement.
  • Assist in care management discussions.
  • Screen and assess program eligibility.
  • Promote health outcomes using case management.
  • Connect clients to medical and community services.
  • Accompany clients to office visits if needed.
  • Advocate for clients and families.
  • Utilize motivational interviewing and trauma-informed care.

Job description

Hours: 40 hours per week; Monday - Friday, 8:00 am to 4:30 pm (no on-call, evenings, or weekends)

Pay Rate: $28.01 to $29.48 per hour

Education: Bachelor’s degree in Public Health, Social Work, Psychology, Human Services or related field with 2 years of experience working in social services or related setting. OR Master’s degree in Public Health, Social Work, Psychology, Human Services or related field with 1 year of experience working in social services or related setting.

Responsibilities
  • Provide mobile outreach and engagement
  • Assist leading care management and case conference discussion
  • Screen and assess for program eligibility
  • Promote positive health outcomes for clients utilizing mobile case management, care coordination with other providers, and supportive services with a team of dedicated professionals
  • Connect clients and their families to a medical clinic and providers, as well as other clinic and community‑based services
  • Arrange transportation and accompany clients to office visits as needed
  • Serve as an advocate for the client and family
  • Use motivational interviewing, trauma‑informed care, and harm‑reduction practices
  • Create and maintain all required documents in the electronic and paper record systems
Qualifications and Skills
  • At least six months of experience working with youth strongly preferred
  • Experience working with Seriously Mentally Ill (SMI) population is preferred
  • A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be a CRF Approved Driver are required to fulfill the essential functions of this position
  • Ability to pass criminal/background clearance
