Job summary

Talentify is seeking a Care Management Supervisor - Adult Programs to lead a remote team based in Santa Barbara, CA. This full-time contract role oversees care coordination for adults with complex needs and ensures HIPAA compliance and high-quality outcomes.

The ideal candidate holds an active CA RN or LCSW license, a related Bachelor's degree, and 5+ years clinical plus 3+ years of supervisory and 2+ years managed care experience. Travel locally may be required.

Qualifications

• Active, unrestricted California RN or LCSW license.
• Bachelor's degree in Nursing, Public Health, Healthcare Administration, Community Health, or a related field.
• 5+ years of clinical experience.
• 3+ years of leadership or supervisory experience.
• 2+ years of managed care experience.
• Strong communication, organizational, and relationship-building skills.
• Valid California driver's license and ability to travel locally as needed.

Responsibilities

• Supervise, mentor, and support Care Management staff.
• Monitor team productivity, performance, and quality outcomes.
• Assist with onboarding, training, coaching, and professional development.
• Collaborate with leadership to improve workflows and program effectiveness.
• Ensure timely communication and coordination among members, providers, community organizations, and internal teams.
• Participate in audits, quality initiatives, and performance improvement activities.
• Support member concerns and escalated case resolution.
• Develop partnerships with community agencies and social service organizations.
• Collaborate with behavioral health teams to enhance care coordination.
• Maintain compliance with HIPAA, confidentiality standards, and regulatory requirements.
